About Records​ ​and​ ​Subpoena​ ​Office

You can submit a work permit application in person or by mail to the address below. Students submitting application by mail must enclose copies of the required documents. Requests submitted without the required documentation will not be processed and will be returned to the applicant.

If applying in person, a work permit may be issued on the same day or dropped off to be picked up within 2-3 working days during hours of operation. If applying through the mail, the work permit will be mailed to the home address of the student.

To request a work permit, you will need to bring to the work permit office:

  • Complete Application: with parent/guardian’s signature at the bottom, employer section completed (especially with “Employer’s Workers’ Compensation Insurance Company” section filled in) and signed by the employer. The application can be picked up at the work permit office, or downloaded from the Pupil Services website.
  • Social Security Card: (photocopy acceptable), OR verification letter from Social Security Administration Office.
  • Proof of Age: birth certificate, passport, OR California ID or Driver License
  • Proof of School Attendance: current report card, current school identification OR dated and signed verification letter with current grades on official school stationery from the school administrator.

Students must renew their work permit with a separate application at the beginning of each school year and each summer. Students must also have a separate work permit for each distinct job.
